The Eleanor Slater Hospital will respond to each inquiry and make every effort to locate a patient record. To speak with a contact service representative, please select from the following list:
- For the Eleanor Slater Hospital – Cranston campus (formerly known as General Hospital - Medical), and Eleanor Slater Hospital, Adult Psychiatric Unit (formerly known as the Institute of Mental Health and Forensic Unit)
This unit retains records of all discharges from 1985 to date. Records prior to this date are no longer available. Master Patient Indices are available. Any requests for a patient’s personal health information should be directed to BHDDH, Medical Records.
