BHDDH Quality Management Unit

In 2019, the Office of Quality Assurance was renamed the Quality Management Unit. This unit encompasses BHDDH's:

  • Office of Quality Assurance

  • Office of Licensure and Standards

  • Office of Quality Improvement

  • Facility Status of Hospitals

The statutory purpose of the unit follows Rhode Island General Laws § 40.1-26-10 and:

     (1) Assure the quality of services provided by agencies or individuals to participants

     (2) Provide for the protection and promotion of the legal and civil rights of participants

     (3) Investigate and evaluate, or cause to be investigated and evaluated, reports made pursuant to § 40.1-27-2.

Quality Management licenses programs that provide services to individuals with an intellectual and/or developmental disability, mentally ill or individuals who have substance use or substance dependence condition.

This Unit processes licenses for organizations that provide Behavioral Healthcare Services, Services for Persons with Intellectual/Developmental Disabilities, and Services for persons with Cognitive Disabilities.

Organizations are issued an 'umbrella' agency license and additional site-specific licenses. The licensure period is for two years. Please click here to find BHDDH Rules and Regulations

The Licensing Office does not process professional licenses for an individual including: Licensed Independent Social Worker, Licensed Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Chemical Dependency Clinical Supervisor, and Licensed Chemical Dependency Professionals. Application Process

Title: Application for Initial Licensure, Renewal, or Add Services/Site Application Form to provide Behavioral Healthcare Services

About: These forms are used for:

  • An organization, not previously licensed by the Department, to apply to provide behavioral health care services (substance abuse and/or mental health) to adults and substance abuse services to minors. The initial form needs to be submitted only once. The 12 licensed services can be chosen from the menu on page 2 (Part III Selected Services Information).
  • Renew an existing license, add a licensed service/program at an existing site, add a new site for licensed service delivery. Licenses are required to be renewed every two years. An “Add Service” or “Add Site” application may be submitted at any time.

Title: Application for Initial Licensure, Renewal or Add a Service/Site  to provide services to Adults with Developmental Disabilities

About: These forms are used for:

  • An organization, not previously licensed by the Department, to apply to become a Developmental Disability Organization which can provide services to adults with developmental disabilities. This initial form needs to be submitted only once.
  • Renew an existing license, add a licensed service/program at an existing site, and/or add a new site for licensed service delivery. An "Add Service" or "Add Site" application may be submitted at any time. Licenses are required to be renewed every two years. Renewal applications must be submitted prior to the expiration date identified on a current license.
