Healthy Transitions And Behavioral Health Guide For Young Adults Healthy Transitions RI Healthy Transitions is a program for youth and young adults, ages 16-25 who are experiencing problems with moods, thoughts, and behaviors, with an emphasis on first episode psychosis. Programs offer access to clinicians, a wraparound case manager, nurse, prescriber, substance use disorder specialist, and an employment education specialist who work as a team to assist participants with meeting personal goals for improving mental health and making successful transitions to adulthood. Youth and young adults will receive help with managing their mental illness through the development of an individualized recovery plan that is developed through shared decision making with the individual. These recovery plans can include treatment goals, goals for housing, employment, education, care, and support. The average length of program participation is two years.
